The following form is a sample contact form using the cgiemail script.
You can insert this form into any of your html pages by copying the source
code for the form.
You should include everything between the <!-- Begin Copy Here --> and
<!-- End Copy Here --> tags.
If you make any changes to the form fields below, you will need to make
corresponding changes to the contact.txt template in your contact
directory before those changes will work.
